12살 몽골소녀의 희망 찾아준 고대병원
어릴적 사고로 성장판 다쳐 왼쪽다리 9cm나 짧아
송해룡 교수팀, 일리자로프 수술로 양쪽 다리 균형 맞출 것
지난달 몽골에서 한국을 찾은 세르겔링(12)양.
4살 때 계단에서 넘어진 이후 별다른 탈 없이 지내던 세르겔링은 7살 무렵부터 왼쪽 다리가 오른쪽다리보다 짧다는 것을 알게 됐다. 세르겔링이 자랄수록 양쪽 다리 길이의 차이는 점점 심해졌고, 특수 제작한 신발을 신지 않으면 걸어 다닐 수조차 없었다.
가족들은 세르겔링을 치료해 줄 저명한 의사를 찾아 러시아, 중국 등 곳곳을 찾아다녔지만, 적합한 의사를 찾지 못했다. 그러던 중 수소문 끝에 고대 구로병원 정형외과 송해룡 교수를 찾게 된 것. 일명 골연장술로 불리는 일리자로프 수술(Ilizarov Treatment)로 매년 200여 명의 환자를 치료하고 있는 등 많은 경험과 노하우를 갖고 있는 송 교수에 대한 믿음이 세르겔링 가족을 한국으로 이끌었다.
일리자로프 수술은 1954년 러시아의 일리자로프(Gavril Abramovich Ilizarov)라는 의사가 개발한 원형의 외고정기를 이용하여 골연장술을 시행한 것에서 시작됐다. 키가 매우 작거나 한쪽의 팔다리가 짧은 경우 또는 외상이나 종양 등으로 인해 뼈 및 연부조직의 결손, 사지 각변형이 있을 때 사용하게 된다. 뼈를 자른 후 하루에 약 1mm씩 늘이는데 서서히 연장해 자른 뼈 사이에 새로운 골이 저절로 생겨나게 하는 방법이다. 환자 개개인의 성장속도를 감안해야하며 너무 빠른 시일 내에 연장하거나 한꺼번에 본인 뼈 길이의 30% 이상을 연장할 경우 연장부위의 뼈가 충분히 자라지 못해 향후 골절의 위험이 높아지므로 장기간동안 전문가에 의해 시술과 관리를 받아야 한다.
지난달 입국해 정밀 검사를 받은 결과, 세르겔링은 왼쪽 무릎뼈(슬개골)가 빠져있었으며, 왼쪽 다리는 오른쪽 다리보다 9cm나 짧은 상황이었다. 송 교수는 일단 슬개골 재건술을 시행했으며, 6월 20일 1차적으로 세르겔링의 대퇴부에 외고정기를 설치했고, 지난 4일에는 종아리 부분에 외고정기를 설치했다. 송 교수는 앞으로 약 6개월을 치료기간으로 잡고 세르겔링의 성장 속도를 지켜보며, 양쪽 다리의 균형을 찾아 줄 예정이다.
세르겔링의 엄마인 캉고즐씨는 “딸 아이가 친구들과 어울려 놀지 못하고 힘들어 하는 모습을 보면서 가슴이 많이 아팠는데, 송해룡 교수님 덕분에 가족 모두에게 희망이 생겼다”며 “아이 왼쪽 다리가 튼튼하게 성장해 다른 아이들처럼 평범하게 걸어 다닐 수 있기를 바란다”고 말했다.
한국의 5인조 남성그룹 SS501을 좋아한다는 세르겔링은 장래희망도 가수다. 치료를 마치고 몽골에 돌아가면 가장 먼저 하고 싶은 것이 무엇이냐는 질문에 망설임 없이 “달리기요!”라고 외치는 밝은 성격의 12살 소녀. 세르겔링이 이국땅에서 오랜 기간 힘겨운 치료를 이겨내고 고국으로 돌아가, 여느 또래 아이들처럼 힘차게 달릴 수 있기를 가족과 의료진 모두가 바라고 있다.
Last month, Sergeling (12/Mongolia), whose left leg was shorter than her right leg, has found us at Korea University Medical Center (KUMC), Guro Hospital. She had been noticing the increasing difference between the lengths of right and left legs. She went to Russia and China to look for a suitable doctor who can treat her, but wasn’t able to find a suitable doctor for her.
They were able to find Professor Song at the Department of Orthopedics of KUMC after inquiries. In the end, the trust on Professor Song, who had the experience and the know-how of treating more than 200 patients every year with the lengthening technique called Ilizarov surgery, led Sergeling’s family to Korea.
The Ilizarov surgery was started by a Soviet physician Gavriil Abramovich Ilizarov in 1954, invented the cylinder-shaped Ilizarov apparatus for lengthening limb bones. It is also used for limb deformity correction or for lengthening a unilaterally short limb.
After the bone is osteomized, the gap between the bones is increased by 1mm a day, and the gap is filled in with new bone materials as time passes by. The patient’s individual healing rate should be taken into consideration during recovery. Because there is a high risk of fracture if the patient lengthens more than 30% or his/her original bone length or the patient tries to lengthen a big amount in a very short period of time, the patient needs to be treated and examined by the specialist for a long time.
According to the close examination done last year, her left knee bone (patella) was dislocated and the left leg was 9cm shorter than the right leg.
First of all, Professor Song performed knee reconstruction surgery, and then an external fixator was installed on the femur on the 20th of last month. On the 4th of this month, another external fixator was installed on the calf. Professor is planning on observing Sergeling for the next 6 months and work on finding the balance between her two legs.
Sergeling’s mother was saying “it was heartbreaking to see my daughter go through such hard time and not be able to hang out with other children. It is good to finally find hope thanks to Professor Song” She added that she hopes that her leg can heal fast and healthy so that she can walk around like normal children.
